All "affected" and "authorized" personnel should be trained on the use of these lockout/tag-out devices. These devices include, but are not restricted to locks, chains and padlocks. Each device is identified and labeled, with the date and time of its application and the name of the person who is authorized to use it and the reason for the tag-out or lockout (repair, maintenance, etc). It is imperative to state clearly that the devices cannot be removed until the LOTO procedure is complete.
